You are invited to participate in a research study that is designed to examine possible causal factors associated with runway incursions involving pilot deviations. Your participation is of great value because your unique perspective as a pilot will help in understanding why runway incursions occur. Your insight will also help in determining how airfield design, marking, lighting, and signage may contribute to these incidents and more importantly how airfield facilities can be improved.

It will take approximately 15-20 minutes to complete this questionnaire. A status bar is provided at the top right hand corner that tracks your progress as you complete the questionnaire. Please note that this questionnaire is part of a doctoral dissertation study being conducted at Florida Institute of Technology's College of Aeronautics, and has been approved by the university's Institutional Review Board (IRB).

Before clicking on "Continue," it is important for you to know the following:

1. Your responses will be confidential and will be accessible only to my professor and me.

2. Your responses will remain completely anonymous and no identifying information will be collected.

3. No reference will be made in oral or written reports that could connect you in any way to this study.
